Alumni Medical Records Request

Stonehill College Health Services keeps immunization records for 10 years from your date of graduation.

If you need a copy of your medical record or immunizations please fax (508-565-1510) or email (Health-Services@stonehill.edu) our office with a letter indicating the following:

  • Name
  • Date of Birth (D.O.B)
  • Year of Graduation
  • Contact Information and Fax # or mailing address of where you would like to have your records sent
  • Phone number where you can be reached
  • Signature
  • Date of Request

Your request will be sent within one week. You may call the Health Services Office at 508-565-1307 with any questions.
